Case Study #3
This college student approached Dr. Morin in Hackensack, NJ, for subtle but meaningful refinement to her nose, enhancing her natural beauty without altering her unique features.
This 19-year-old college student came to Dr. Morin with concerns regarding the appearance of her nose. Specifically, she was unhappy with her dorsal hump, drooping tip, bulbous tip and overall long nose. In addition, she desired a more defined and symmetric nasal tip. Her goal was to improve the aesthetic appearance of her nose without changing the overall beauty of her face.
After a thorough evaluation, it was determined that to achieve the patient’s goals, the nasal bones would need to be addressed first. A dorsal hump reduction would be performed to create a straight nasal dorsum. Nasal bone infracturing would then be performed to create a narrower nasal dorsum. Attention would then turn to the nasal tip, which would require both support and definition. The septum would then be straightened in order to create symmetry. Finally, septal cartilage grafts would be placed in order to provide long-lasting structural support.
Surgery took place in the summer during her break from college. By performing surgery in early August, Dr. Morin was confident that she would be in good shape to return to her university in the fall. He used an open rhinoplasty approach as he generally does when addressing major issues involving the nasal tip. He began the surgery by reducing her dorsal hump and infracturing her nasal bones to create smooth, straight and symmetric dorsal aesthetic lines. He then straightened her nasal septum to ensure her nose remained precisely in the midline. A septal extension graft was carefully created from the patient’s own septal cartilage and securely attached to the septum to provide strong support for the nasal tip. The tip was then refined with a cephalic trim, and additional definition and support were achieved using tip-defining sutures and alar rim grafts.
Surgery went beautifully, and the patient did well during her postoperative recovery. She was called the night of surgery and reported no pain. She was seen in the office the next day, and her intranasal packing was removed. She was seen again one week later for removal of the sutures and external nasal splint. Tape was placed on her nose for one more week to further reduce swelling. The patient was then seen again at one month, six months, and one year following surgery. At one year, the result was incredible. Her nose was straight, the hump was gone and the tip was defined.
The patient was extremely happy with her experience and thrilled with her results. Her dorsal hump and drooping tip were no longer a distraction, therefore her natural beauty was truly allowed to shine.
